on the Southwest corner of the VA grounds,
just West of the Milwaukee VA Medical Center.

Most people choose to take Interstate 94 to Miller Parkway.

• Take I94 to Miller Parkway
• Turn South on Miller Parkway
• Turn West on National Avenue for 0.4 miles
• The office is on the North side of National Avenue
• Visitor parking is available in front of the office

The Milwaukee VA Regional Office is also on the
Milwaukee County Transit bus system, Route 18.

Requests for Records

To request a copy of your records, please write to us indicating the specific records you'd like.  Then send your signed request to us in one of the three following ways:

• Mail your request to the appropriate address shown on the right side of this page.  Requests related to Pension and Survivor Benefits records should be sent to the Pension Management Center (PMC).  Requests related to Fiduciary matters, should be sent to the Fiduciary Hub.  All other requests should be sent to the main Regional Office address.
• Fax your request to the fax number listed on the PMC or Fiduciary Hub pages of this site.
• Email your scanned copy to our FOIA email box.

In general, whereas the Freedom of Information Act (FOIA) ensures government transparency through the release of information, the Privacy Act of 1974 protects the privacy rights of individual citizens by only letting Federal agencies release records with personal information to the Veteran or dependent about whom the records refer. 

• If you would like to give another person permission to receive a copy of your records, please complete VA Form 3288, Request for and Consent to Release of Information from Individual's Records.
• There are also a number of "Routine Uses" posted in the Federal Register, that allow us to provide records to other parties.
