Converting Family Servicemembers' Group Life Insurance Coverage

FSGLI spouse must be converted within 120 days of: service member’s separation from military, divorce, member’s election to cancel their SGLI or FSGLI spouse coverage, death of the service member.  Child coverage cannot be converted.

You may choose one of the participating conversion companies.  You will need to provide the company with your FSGLI conversion notice, a copy of the Service member's Final Leave and Earnings statement, PLUS one of the following:

  • Service member's DD-214, NGB-22, or written orders as proof of separation from the military,
  • Service member's death certificate, or
  • Proof of Service member’s termination of their SGLI or Family SGLI spouse coverage
